Information about Suhadolnik's research on a bio-marker has been added to Section
1.03, and Section 2.05 now includes references to Suhadolnik, the Sydney, Australia
research team, and CBT work being done by Wessely, Sharpe, et al. "CBT has been
added to the abbreviations in Appendix 2.
