This article is from the Childhood Vaccinations FAQ, by Lynn Gazis-Sax lynng@alsirat.com with numerous contributions by others.

health care providers, vaccine manufacturers, and vaccine recipients
(or their parents/guardians). Patients, parents, and guardians are
encouraged to seek the help of a health-care professional in reporting
to VAERS.
